Background

Orthokeratology may now be best known for myopia management, but its original purpose was to provide freedom from daytime contact lens wear. This case is a reminder that ortho-k still offers an elegant solution to many of the everyday problems associated with conventional methods of vision correction.

Patient History

A 28-year-old female patient attends the consultation with a diagnosis of myopia for more than a decade. Her lifestyle is highly demanding: professionally, she spends long hours in front of the computer, and her main hobby is Fitness Training. She reports dissatisfaction with her glasses, as they limit her sports performance, and occasional use of soft lenses caused discomfort at the end of the day due to her high digital workload.

She reports no personal medical history.

RECOMMENDED MATERIAL:

For this case, the Optimum Infinite material from Contamac was specifically chosen over 100 Dk options for the following critical clinical reasons:

    • Night-time Safety: As this is a nocturnal treatment (eyes closed), the cornea depends exclusively on the oxygen passing through the material. A Dk of 180 ensures maximum oxygen flow, minimising any risk of corneal oedema or hypoxia.
    • Digital Lifestyle: Given that the patient works extensively in front of screens and already presented a BUT of 8s, we needed a material that would maintain corneal physiological integrity without worsening dry eye.
    • Corneal Safety: Using an Orthokeratology design, choosing Hyper-Dk (180) was a proactive decision to ensure a clear and healthy cornea not only short term, but throughout years of continuous treatment.

Topography post-treatment revealed a perfect oblate pattern with a centred treatment zone in both eyes. Corneal health remained impeccable, with no staining or anterior segment alterations, demonstrating the effectiveness of high oxygen transmissibility.

Conclusion

The use of Optimum Infinite turned this case into a resounding success. Not only did we achieve the desired visual correction, but we also ensured long-term ocular health.
